Physician Resources: Requisitions, Requisition Pads, Portal Access, and Reports

How can referring physicians and clinic staff send requisitions, access reports, request requisition pads, or use the GNMI physician portal?

Overview

This article is for referring physicians, clinic staff, and administrative teams who need to send patients to GNMI or access physician resources.

Patients need a valid imaging requisition before most exams can be booked. The requisition can come from any licensed physician or authorized healthcare provider. It does not need to be on a GNMI-specific requisition form.

What You Need to Know

  • A valid requisition is required for imaging tests.
  • The requisition does not have to be on a GNMI form.
  • Requisitions can come from any licensed physician or authorized healthcare provider.
  • GNMI provides physician resources for requisition forms, requisition pads, portal access, and portal login.
  • Patients can ask reception to send their report to another physician.

For Referring Physicians and Clinic Staff

Physicians and clinic staff may need GNMI resources for:

Requisition Requirements

A requisition is the medical order that tells the imaging clinic what exam has been requested and why.

Before sending a patient, make sure the requisition is complete and clear. GNMI should confirm the exact required fields before this article is published, but a complete requisition usually includes patient details, requested exam, clinical history or reason for exam, referring provider information, and signature.

Patient Booking Notes

Patients who already have a valid requisition can book an appointment through GNMI’s booking process.

Standard X-rays are walk-in, while ultrasound, MRI, CT, nuclear medicine, mammography, BMD/DEXA, and other specialized exams may require a booked appointment.

Reports and Results

Imaging results are sent to the referring physician or authorized healthcare provider.

If a patient wants their report sent to another doctor, they should tell reception. The process may depend on GNMI’s current report-sharing policy and privacy requirements.
